Reports

Reports

Reports are used for many reasons; a report written by one of our speech and language therapists can be used as a second opinion, to inform schools or other professionals about your child’s difficulties, or a summary of an assessment and its findings. We also write reports for case managers and solicitors, all of our reports identify children’s strengths and weaknesses in speech, language and communication, as well as recommendations from the speech and language therapist.

The purpose of children’s speech and language therapy reports

Speech and language therapy reports are available upon request and written specifically to outline your child’s needs. Some of the common reasons for requesting a report are to:

  • Receive a hard copy of the findings of the assessment completed and any recommendations or advice given.
  • Summarise a completed assessment.
  • Record a specific difficulty and its requirements across various settings and situations.
  • Justification and provision of aids and adaptation of environment.
  • Provide a clear description of the recommendations for therapy.

Available paediatric speech and language therapy reports

We offer the following speech and language therapy reports that are available following a speech and language therapy assessment.

Detailed report

A detailed report can be provided after assessment or block of therapy to detail findings or progress. They can also be written upon request in order to support any diagnosis being made.

Following a swallowing assessment it is important to detail any findings and recommendations in a report that can be passed on to anyone that will find this information useful when dealing with the child’s eating and drinking.

Education, Health and Care plan report

Our speech and language therapists can write a detailed report on their findings from assessment and therapy detailing goals for the child in relation to how an EHCP will support achieving these goals.

Medico-legal report

A medico-legal report can be written upon request that will detail findings from assessment in terms of quantity and price of therapy required after an accident or medical negligence.
