Dysphagia report
A dysphagia report gives details about a child’s eating, drinking and swallowing skills, as well as recommendations to ensure the child is safe when eating and drinking.
You may choose to use our report to provide yourself and others involved with your child about the safest practise when your child is eating and drinking, to avoid aspiration or choking.
The report will contain the following:
- Reason for referral
- Background information (include medication, current health status)
- Summary of assessment
- Areas of risk
- Recommendations e.g. posture, environment, oral hygiene, sensory and behavioural issues.
- Treatment plan
The report will also provide long term and short term goals and how these may be achieved and reviewed.
